GSK - Nicorette Reward

Program Terms:

Purchase NicoDerm CQ or Nicorette products from Walmart or Walmart.com in a single transaction between 4/1/20 through 6/30/20

Store Receipt/Shipment Confirmation email must show retailer name, date, time, receipt total and qualifying purchase.
Each Store Receipt/Shipment Confirmation/receipt can only be submitted once.
Limit of one (1) receipt per email address/household for the duration of the program.

Place a check mark next to the following information in your receipt before taking your photo:

Participating Store
Date and Time
Participating Purchase
Receipt Total

No scanned submissions will be allowed. Snap a picture of your receipt ensuring all of the items above are in view and upload the photo to StartYourQuit.com by 7/10/2020 at 11:59:59pm PT (All submissions subject to verification).

Tips on Taking Your Photo
Flatten the receipt and place it on a dark non-reflective surface. Ensure that there is enough light and the entire receipt is in the frame. When taking the picture, make sure the receipt is in focus. Check the image quality prior to submitting.

After placing an order on Walmart.com, you will need to wait for the email that confirms your items have been delivered. Please note, the shipment confirmation email may not arrive until several days after you have placed your online order.

Once you receive your order, submit your shipment confirmation by clicking the “Submit online receipt” button above. Please be sure to enter a valid email address. All email responses (including the eGift Card) will be sent to the email address that you provide.

Please note that the email simply confirming your order will not be accepted. You will need to submit a PDF of the shipment confirmation email, which may not arrive until several days after you have placed your online order.
